I would guess the firing pin hole in the breech face is sharp or may have a slight burr on it.
This was the issue.
He used some high grit wet sandpaper and problem solved.

I haven't experienced anything like that. I agree there has to be something "tearing" at the brass casing. What does the back of the casing look like? For those pieces to get into the strike channel, I would have to agree that the firing pin hole is the likely culprit. The easiest way to check for ...

Cool, man, thanks for the info. Did you have any issues before you put the FS trigger in? YW ! I had it once or twice but at that time i didnt think of it. Just thought it was me doing something wrong. The issue start directly after i put the FS on and it was a constant fail when i racked the slide...

Just checked my 100% stock SFx and it and seems to behave exactly like yours. My thoughts on the symptom you describe seems similar to what mine did when new.
